Identifying Patient Concerns
To ensure processes are in place for continuous quality improvements, we need ongoing feedback and identification of patient safety concerns throughout the hospitals. Historically, the explanation of how and why medical errors and adverse events occurred focused on individuals and not the system at large.
To have the process standardized in a Quality and Patient Safety plan, and monitor the comfort staff have in reporting concerns will add to a culture of patient safety within the organization.
To develop the Plan, we will survey staff to understand the current culture as a benchmark to measure their comfort to speak up to identify a patient safety concern, and set a target for improvement.
